Job Summary
A qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. Care is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse.
This position is required to take on-call shifts on nights, weekends and holidays as assigned.
Accountabilities
Implements the nursing care plan for each patient. Provides nursing services, treatments, and diagnostic and preventive procedures according to the plan of care as assigned. Initiates preventive and rehabilitative nursing procedures as appropriate. Prepares equipment and materials for treatments. Assists the RN in performing specialized procedures. Observes aseptic technique as required. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Evaluates with the registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse. Maintains accurate and complete records of observations, treatments and care of patient. Prepares clinical and progress notes. 30%
Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Assists the patient in learning appropriate self-care techniques. 30%
Participates in patient care conferences and discusses the need for involvement of other members of the health team, such as physical therapist, speech therapist, occupational therapist, and medical social worker. 20%
Submits any changes in schedule to clinical manager on a daily basis. 5%
Understands and adheres to established policies and procedures. 5%
Attends staff meetings, patient care conferences, and in-services as scheduled. 5%
Participates in medical record audits as assigned. 5%
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Completion of a LPN program recognized by the SC State Board of Nursing.
One year experience as a LPN.
Experience in Home Health preferred.
Required Certifications/Registrations/Licenses
Licensed to practice as a LPN in South Carolina
SC Driver's license
In Lieu Of The Above Minimum Requirements
In lieu of 1 year of LPN experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency program from another organization.
In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing.
In lieu of 1 year of LPN experience, will accept candidates enrolled in an accredited LPN to RN transition program.
Other Knowledge, Skills or Abilities
Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
Ability to exercise initiative and independent judgment required.
Acceptance of philosophy and goals of this agency required.
Guidelines for Changes in LPN Licensure
If an LPN team member working in this position obtains RN licensure, Prisma Health will accept RN licensure for a limited time until the team member can be placed in an RN position. Team members should immediately notify their manager and Talent Acquisition if they are scheduled to sit for the NCLEX-RN exam or have obtained RN licensure.
