Responsibilities:
-Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:
A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
B. Assesses for muscle strength, mobility, gait, ROM-potential for rehab.
C. Directing physical therapy treatment.
D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
E. Determining priority needs for physical therapy
- Evaluate and assess patients' physical conditions and develop appropriate treatment plans
- Implement therapeutic exercises and techniques to improve patients' mobility, strength, and range of motion
- Provide manual therapy techniques such as trigger point therapy to alleviate pain and promote healing
- Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.
- Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
- Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
- Educate patients and their families on proper techniques for managing their conditions and preventing further injury
- Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
- Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.
- Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
- Maintains necessary records; Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy (within 48-hours)
- Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.
- Maintain accurate and up-to-date patient records
-When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.
- Participates in peer consultation process.
- Supervises Physical Therapy Assistants according to organization policy and state regulations.
Experience and Qualifications:
- Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.
-Licensed to practice as physical therapist within the State, if applicable.
- Possesses and maintains current CPR certification.
- Current certification or license as required by state law.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Minimum of 2 years of experience as a Physical Therapist
- Strong knowledge of anatomy, physiology, and medical terminology
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to work independently and make sound clinical judgments
- Proficient computer skills for documentation and data management
- Experience with Synergy EMR is preferred but not required
- Experience documenting accurate OASIS data is preferred but not required
- Understanding of the PDGM Medicare model
Skills:
- Excellent patient care skills, including empathy, active listening, and effective communication
- Proficiency in conducting thorough evaluations and developing individualized treatment plans
- Ability to adapt treatment techniques based on patients' needs and progress
- Strong manual therapy skills, including trigger point therapy
- Knowledge of evidence-based practice guidelines and ability to integrate them into treatment plans
- Experience working with geriatric patients is a plus.
