Supported Aging Program Position
The Supported Aging Program Supervisor is responsible for working with staff and Members to plan, research, and facilitate the new Supported Aging Program - focusing on people with I/DD over 50 and/or who have a diagnosis or are exhibiting symptoms of Alzheimer's/Dementia. The Supported Aging Program Supervisor will focus on assessments, interventions, program upgrades, and supplies necessary to provide the proper interventions and supports to assist our Members as they age in place.
- Conduct tours, coordinate intakes, and complete initial evaluation of persons referred to and accepted by The Opportunity Tree's Supported Aging Program in collaboration with site teams.
- Responsible for accurate and timely completion and reporting of program performance metrics.
- Collaborate with Service Coordinators to design and implement assessments and evaluations.
- Develop, implement, and evaluate Supported Aging metrics for individual Members in their Person-Centered Service Plan (PCSP).
- Create and implement Supported Aging Curriculum at all sites.
- Supervise Direct Support Professionals.
- Coordinate, facilitate, and oversee Creative Aging scheduling, attendance, and reporting.
- Work collaboratively and effectively with Site Managers and others for direct or indirect reports and wants/needs for the program.
- Research and identify potential funding and partnership opportunities to sustain the program.
- Foster community partnerships in relation to Supported Aging Program.
- Maintain communication with Hospice of the Valley throughout the duration of project.
- Ensure appropriate reporting and compliance with state and federal contracts and all grant requirements – in collaboration with the CEO.
Requirements:
- Minimum of 18 years of age.
- Current driver license with Non-Restricted Motor Vehicle Record and compliance per the requirements of The Opportunity Tree's insurance carrier.
- Excellent communication skills, including the ability to write, speak, and listen effectively in English.
- Experience: Two (2) years in the Intellectual and Developmental Disabilities (IDD) field in a vocational, rehabilitation, or similar setting preferred.
- Education: Bachelor's degree preferred.
- Knowledge: Intermediate in MS Outlook, MS Word, MS Excel, web browsing.
- Knowledge of best practices in adult day programs and vocational training programs preferred.
- The ability to utilize data to assist in making financial, strategic, and programmatic decisions.
- Knowledge of best practice models and community integration.
- Positive attitude and represent The Opportunity Tree in a positive manner to the community.
- Successful completion and maintain all required training which may include CPR, First Aid, and Prevention and Support.
- Successfully meet AZ-DPS requirements for Fingerprint Clearance Card.
- Reliable transportation and travel for The Opportunity Tree business.
