Job Description:

Patient Care: Provide comprehensive care to patients with a variety of medical conditions, including monitoring vital signs, administering medications, and performing wound care.
Assessment: Conduct thorough assessments of patients, including physical exams and patient history, to identify health issues and determine appropriate interventions.
Care Planning: Develop and implement individualized care plans in collaboration with the healthcare team to address patient needs and goals.
Monitoring: Continuously monitor patients' conditions, including detecting and responding to changes in vital signs, symptoms, and overall health status.
Patient Education: Educate patients and their families about their medical conditions, treatment plans, and discharge instructions, promoting self-care and health management.
Documentation: Maintain accurate and detailed records of patient assessments, treatments, progress, and outcomes in electronic health records (EHR).
Collaboration: Work closely with physicians, specialists, and other healthcare professionals to ensure coordinated and effective care delivery.
Emergency Response: Recognize and respond to medical emergencies and complications, providing appropriate interventions and coordinating with emergency services as needed.
Infection Control: Adhere to infection control procedures and protocols to ensure a safe and clean environment for patients and staff.
Discharge Planning: Assist with discharge planning, including arranging follow-up care, providing patient education, and ensuring a smooth transition from hospital to home or other care settings.
