Wound Care Nurse- Skilled Nursing CommunitySprings Village Care CenterColorado Springs, CO The primary responsibility of your job position is to provide excellent nursing care to patients suffering from wounds, evaluate wounds and injuries for infections or other illness factors that causes wounds and direct nursing care to residents and to supervise the day-to-day nursing activities following the current federal, state, and local standards, guidelines and regulations that govern long term care and skilled nursing facilities. You are entrusted to ensure that the highest degree of quality care will always be provided to our residents.

Essential Duties and Responsibilities: - Communicate changes in resident's condition with employees, physician, resident and family members.
- Perform administrative duties such as: completing medical documents, reports, evaluations, studies, charting etc.
- Admit, transfer and discharge residents as required
- Complete accident/incident reports
- Assure adequate supplies are available including but not limited to pharmaceuticals, resident personal supplies and equipment
- Assure that resident abuse does not occur
- Complete rounds to assess, develop and implement various types of care needs on each resident including but not limited to: wound care, behavior management, respiratory care etc.
- Perform all other duties, as assigned
Qualifications: - Experience with wound/care ostomy. Will provide ability to become certified.
- Current, unencumbered, active license to practice as RN in Colorado, Current BLS/CPR Certification
- Ability to pass a criminal background check as well as Colorado CAPS background check.
Job Summary:
- Performs skills (wound care) accordingly and without unnecessary harm/pain to the patient.
- Assesses patient condition, initiates plan of care, re-evaluates and updates, as necessary.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques.
- Educates the patient and family regarding pain management.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
- Obtains all necessary physician orders and collaborates with the physician as the patient dictates.
- Performs patient care responsibilities considering needs specific to the standard of care for patients age, primarily geriatric.
- Communicates patient's data and observations verbally and through documentation in Point Click Care (PCC)
- Demonstrates knowledge of medications and their correct administration based on age of patient and patients' clinical status.
- Teaches, supervises, and counsels the patient and family members regarding nursing care needs.
- Follows-up with obtaining lab results and notifies the physician of any changes.
- Demonstrates appropriate universal precautions and infection control measures.
Skill/Education Requirements:
- Current Colorado Registered Nurse license.
- Current BLS certification.
- Certified in Wound Care and/or eligible for certification.
