Job Summary of a Licensed Practical Nurse: To provide quality of life and care to residents under the general supervision/delegation of a registered nurse according to established policies, procedure and standards of practice and care. To provide motivation and monitoring of staff performance. When working in the Charge Nurse capacity is responsible for assisting in the operational functions, management, and clinical activities of the department.

Key Essential Functions of a Licensed Practical Nurse: • Data Collection: Performs initial data collection on all residents and follows up on date collection per GSLS policies and procedures. • Implementation: Administers, delegates, and monitors quality direct resident services to assure quality of care for all residents in accordance with the privileges and limitations of the Nurse Practice Act. • Medication Administration: Demonstrates knowledge of medications and their correct administration based on each resident's status, diagnoses, and medication regimen. Observes, documents, and reports resident responses to medications. Is knowledgeable and follows medication policies and procedures. • Treatments and Clinical Skills: Competently performs skin treatments, catheterizations, tube feedings, and other treatments and cares as needed. • Documentation: Documents professionally, accurately, and completely according to current standards, and GSLS policies and procedures. • Discharge Planning: Plans for resident discharge, collaborates with the Registered Nurse and discharge planners for long- and short-term resident needs. • Emergency and Disaster Measures: Carries out first aide, emergency, and disaster measures according to policies, procedures, and standards of care.
Physical Demands of a Licensed Practical Nurse: • Regularly required to stand, bend, and walk. May also be required to sit, climb, stoop, kneel, crouch, push and pull. • Regularly assigned to lift or move objects up to 10 pounds. • Frequently required to support up to 10-50 pounds. • Emergency situations could require carrying/supporting 25 pounds or greater. • Requires employee use hands for repetitive actions such as simple grasping, firm grasping and fine manipulating. • Requires functional hearing ability, read medical records, make observatory assessments, and document in writing and computer records. • Requires functional ability to perform job duties as listed in the job description.
Qualifications of Licensed Practical Nurse: A. Education/Other- • Graduate of a licensed practical nurse accredited program required. • Current active Licensed Practical Nurse (LPN) nursing license in the State of Minnesota in good standing. • CPR certified. • Must be able to read, write and communicate in English. • Clinical experience with geriatrics, long-term care and/or rehabilitation preferred. • Demonstrates a broad and current knowledge of nursing including gerontological nursing and related aspects such as rehabilitation and metal health. • Knowledge of and skilled in physical assessment techniques to gather data. • Skilled in oral and written English communication-communicates clear expectations. • Knowledge of Quality Improvement-welcomes change to improve.
