We are seeking a full-time Social Worker to provide supportive services for pregnant and parenting people who have a history of substance use through culturally appropriate, community-based, multi-generational case management and sober support group facilitation.
In this role you will provide comprehensive case management services directed toward pregnant and parenting people with substance use concerns. You will collaborate with community service providers to decrease the number of babies born exposed in-utero to substance use by their birth parent. Ensure program success by engaging families to enhance safety, well-being and stability for children while improving parental self-sufficiency and mitigating risk factors concerning substance use/misuse. You will do this by:
- Conducting client assessments to identify strengths and barriers that affect family stability such as employment, housing, health care and other social/emotional challenges that can impact child safety and well-being.
- Creating individualized case plans for guidance and support focused on goal setting in the areas of sobriety, pre-natal and post-natal care, parenting skills and child development. Facilitate access to needed resources in accordance with the case plan to enhance self-sufficiency for the family.
- Coordinating services with Child Protection, Public Health, Community Corrections, Mental Health/Chemical Dependency treatment providers and other relevant community service providers.
- Engaging in outreach activities to assist with building and maintaining a network of resources while advocating for families.
- Co-facilitating weekly or bi-weekly sober support and education groups as well as coordinating and host parent/child activities as scheduled.
- Completing all required documentation in a timely manner, which may include time reporting, case notes, and family support plans.
Our ideal candidate possesses excellent critical thinking skills and the ability to thrive in a team-led environment. A bachelor’s degree in a human services related field is required. Preference will be given to those with experience with substance use, child welfare, direct work with diverse populations and bi-lingual skills. Preference will also be given to those with a Minnesota Social Work License or license eligibility.
