We are actively recruiting for Certified Nursing Assistants to work with our Day Center team. This position requires a dynamic individual who enjoys working in an environment that encourages developing relationships with our participants and their caregivers, where a team focused approach to care is the cornerstone of our organization.
Job Summary: The Certified Nursing Assistant performs participant care activities and other tasks as assigned to assist the primary care, therapy, and adult day health staff. Works under the direct supervision of the Day Center Supervisor or Home Care Supervisor, dependent on position.
Key Responsibilities
• Communicates effectively with participants, families, staff, and the staff of other agencies.
• Works effectively in a team setting.
• Provides basic ADL nursing care to participants in the PACE Day Center or participant home
• Reports changes in the participant's condition to the pertinent Interdisciplinary Team member (IDT).
• Demonstrates active participation in Quality Assurance processes.
• Participates in the process of teaching and learning with others.
• Abides by all established PACE of Southwest Michigan policies, rules, and regulations.
• Clean facility equipment & maintain clean facility conditions and report on participant status or family concerns to the CNA
Lead or Day Center Supervisor).
• Conducts SBAR's as needed and reports findings to Clinic Lead.

Key Responsibilities
· Provide assistance, support, and/or guidance for participants in their home environment as directed
· Travel to participant homes to provide care
· Assist with all ADLs as trained appropriately and directed
· Assist with two-person assist transfers, turning, positioning, and transferring participants into and out of bed, chair, shower, wheelchair and use of mechanical lift as needed
· Obtain monthly/weekly weights as directed
· Communicate any changes in function or behavior to the Home Services Supervisor
· Assist participants on/off the bus at their homes
· Participate with Quality Improvement activities
· Assist with housekeeping tasks in the home, to include but not limited to – sweeping, mopping, laundry, grocery shopping, etc. as directed.
· Assist with personal care tasks in the home, to include but not limited to – bathing, toileting, changing, dressing/undressing, etc. as directed.
· Work collaboratively with all other departments to ensure that the needs of participants are consistently met and that a clean, sanitary, and safe environment is maintained at all times
· Build and maintain trusting relationships with participants
· Identify/report safety hazards and emergency situations and initiate corrective action as appropriate
· All other duties as assigned.

Requirements
Education: High school diploma or general education diploma.
Experience: Six months clinical experience as a personal care aide, nursing assistant, or emergency medical technician.
Licensure, Registry or Certification Required: Certified in MI as a CNA, Valid MI Driver's License with good driving record, CPR certification.
