Under general supervision, actively incorporates our "Philosophy for Person-Directed Care." Develops awareness of residents' interests and needs, and assists residents to maximize their rehabilitation, independence and participation. Will provide care and assistance of residents in the Skilled Nursing arena and on limited occasions may supervise caregivers providing resident services; effectively plans and implements nursing services to provide quality care.
Pay: $30-$35/hr depending on experience
Plus $1-$3 shift differential dependent on shift/day working
Schedule: 6pm-6am, Fridays with potential for other shifts.
REQUIREMENTS:
Education:
- Graduate of an accredited nursing school and currently licensed as an LVN/LPN to practice in applicable state.
Experience/Training:
- Senior Care, Skilled Nursing, Hospital or Rehabilitation Experience
- Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members.
- Minimum of 1 year of professional nursing or caregiver experience preferred.
Certificates, Licenses, Registrations:
- Current LPN license in Oregon
- Current CPR certification
Work Duties
- Provides resident care, including administering medications and documents each treatment and maintains weekly skin condition reports
- Maintain communication with doctors, laboratory, pharmacy and residents' families concerning resident care;
- Attend care plan meetings;
- May lead caregivers or direct some caregiving services;
- Conducts admission assessments as well as assessments for change of condition under the direction of the RN
- Performs health updates, interviews, and charting; maintains daily records, takes doctors' orders;
- Prepares shift reports;
- Provides effective and courteous service to all residents, guests, and co-workers;
- Conducts work tasks safely and in compliance with the facility safety program;
- Answers telephones and assists visitors;
- Attends in-service training workshops, seminars and meetings, as required;
- Promotes and protects the rights of each resident;
- Performs other related essential duties as required

This position is based onsite at our Hillside Community in McMinnville, Oregon
