Description:
Envision Unlimited is a large non-profit agency committed to providing services for adults with intellectual/developmental disabilities and/or mental illness. We emphasize quality services that promote choice, independence, and community integration. Through the collaborative efforts of an interdisciplinary team, our members receive person-centered services aimed at improving the functionality and practical application of social, financial, and emotional skills. In addition, we work to build partnerships with our community to enrich the lives of our members by creating lasting opportunities that will strengthen relationships for a lifetime. Our programs include, but are not limited to, Community Day Program, Community Living, Mental Health, Employment and Career Services, and Foster Care.
Purpose: To provide direct casework services to persons with developmental disabilities in their homes and in the community. (Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions).
Essential Responsibilities:
1. Coordinate the completion of service agreements and service authorizations.
2. Work with the family to write the Employer of Record Implementation Strategy.
3. Maintain client files in the Quantum electronic records system.
4. Maintain accurate and concise case records and progress notes on each member, keeping them current.
5. Complete other reports/forms as assigned.
6. Seek out referral sources in the community and effect linkage when appropriate.
7. Work in collaboration with other staff and programs regarding the needs of shared members when necessary and appropriate.
8. Accompany members to referral services when indicated and act as advocate on members' behalf.
Other Responsibilities:
1. Assist supervisor in special projects.
2. Meet weekly with supervisor concerning cases.
3. Maintain an accurate work schedule and accurate mileage records.
4. Perform other related responsibilities as assigned.

Requirements:
QUALIFICATIONS
To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
EDUCATION, SKILLS, and/or EXPERIENCE
- Bachelor's degree in human or social Service field.
- At least one year of direct experience working with adults with DD (Developmentally Disabled Population).
- Must be QIDP (Qualified Intellectual Disability Professional) eligible upon hire.
- Must have a valid driver's license.
- Must have use of a reliable car to travel throughout the Chicagoland (Cook County) area.
- Must be willing to travel up to 70% of the time, and the other 30% will be working from the Rose Center office (gas and milage reimbursement provided).
Schedule: Generally Monday-Friday 9am-5pm with flexibility when needed. This job requires candidate to travel to individuals homes throughout the Chicagoland (Cook County area).
Other Responsibilities:
Must complete 12 hours of continuing educations units (CEU) annually.
Participate in variety of committee and staff meeting groups.
Participate in quarterly quality assurance reviews.
Attend and assist at agency and member events.
Perform other duties, as assigned.
