Outpatient Clinic located in Buckhead/Atlanta is seeking a dedicated Rheumatologist to provide physician services to include patient and clinician consultation, primary care management of people with primary care management of people with chronic diseases, and additionally functions as a part of a large, integrated care team dedicated to providing whole-person care in outpatient setting.
Those with HIV infection are at increased risk of autoimmune and connective tissue disorders such that seamless access to the rheumatology specialty enhances clinical outcomes, quality of care, and patient experience. Others in primary care may suffer primarily from rheumatologic disorders, with immediate access to the specialty and continuity of care dramatically improving outcomes. The clinic has invested in state-of-the-art diagnostics, including an upgraded dual-energy X-ray absorptiometry (DEXA) scanning platform offering trabecular bone score capability for improved fracture risk assessment. Substantial clinic laboratory assets allow for in-house serologic studies and future deployment of evolving molecular targeted assays important for advanced rheumatologic diagnosis and care.
In day-to-day care, the Rheumatologist will conduct outpatient consults based on internal and external referrals, follow a panel of patients with chronic rheumatologic disorders, develop diagnostic and treatment care plans, counsel patients as appropriate, provide educational opportunities for peer clinicians, while collaborating to develop the clinics specialty services and cultivating opportunities for service promotion and community awareness.
Essential Duties/Responsibilities: - Provide care for people living with chronic rheumatologic conditions including inflammatory and degenerative arthritides, autoimmune diseases, osteoporosis, and related disorders.
- Conduct outpatient care consultations, creating diagnostic and treatment care plans.
- Provide clinician consultation services, chiefly to internal clinicians but also to collaborating practices and external clinical entities.
- Develop an intensive and comprehensive model of outpatient care for patients with complex chronic diseases, collaborating with clinic leaders, other specialists, and care team members.
- Practice evidenced-based medicine in accordance with the PCMH model of care.
- Supervise and collaborate with assigned advanced practice providers (APPs, including NPs and PAs) as necessary to extend access to quality care for clinic patients.
Required Skills/Abilities: - Knowledge or experience with NCQA PCMH model is preferred.
- Must be able to work as part of a multidisciplinary team with constant collaboration within and across provider teams.
- Excellent interpersonal and communications skills.
- Effectively adapt and respond to a complex, fast-paced, and results-oriented environment.
- Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities.
- Working knowledge of Microsoft Office Suite (Word, Outlook, PowerPoint, and Excel).
Education and Experience: - A medical degree from an accredited university required.
- Licensed physician in Georgia
- Board certified following specialized training
- American Heart Association BLS certification required.
- Experience in population health management is preferred.
- Experience in working with psychosocially and medically complex patients is strongly preferred.
