SUMMARY
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors are pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining fidelity to the model.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Shares resources, performs health checks, and partners with first-time expectant parents during pregnancy, and until the child is two.
- Community-based position. Travel to and visits occur in home or community location.
- The NFP nurse works with families to uncover their innate strengths and skills as parents.
- The NFP Nurse will carry a caseload of 25-30 clients.
- Extensive model training by the NFP National Service office and ongoing reflective supervision with site supervisors. Individuals must be compassionate nurses with experience in maternal health, labor and delivery and community health nursing.
- Travel required visiting families, attending training and meetings, and adhering to company and program guidelines.
- Position covers travel in Kent and Sussex County
- Ability to work flexible hours, including nights and weekends, as needed.

EDUCATION, LICENSURE and EXPERIENCE
- Eligible candidates will need RN, with a BSN, at minimum, and DE license.
- Must be a registered nurse with a minimum of a Bachelor's degree in Nursing (BSN) with a current RN license (in good standing).
- Experience in public health, community, and/or maternal-child health, required.
- Current CPR required.
- Home visiting experience preferred.
- Bi-lingual (English/Spanish), desirable.
- Must have a valid driver's license, which meets the requirements of the agency's insurance carrier.
