The Opportunity: Care Coordinator, Hello House for Men
Job Type: Part-time, Weekends (16 hours)
Pay: $25/ hour

As Care Coordinator, you'll have a dynamic and critical role working to provide a high quality of behavioral health care at a residential co-occurring recovery home. You will work with individuals diagnosed with co-occurring disorders( mental health and substance use disorder) and provide care management and support. As Care Coordinator, you will support clients in accessing treatment services and community resources.
Some of your critical responsibilities:
- Engage with clients following admission to help individual's access treatment services within VOAMASS and externally with community-based providers.
- Engage and build rapport with clients, assist clients with referrals and coordination to services and resources including primary care, mental health treatment, addiction treatment, recovery support, employment specialists, housing specialists and other social supports.
- Provide follow-up assessments and referral providers, to ensure that clients attend appointments and access treatment.
- Establish and maintain good working relationship with referral agencies to ensure continued coordination.
- Serve as part of the integrated services care team and multidisciplinary to review team to enhance client experiences and maintain agency compliance with state regulations and mental health professional standards.
- Maintain records according to VOA Policies and Procedures, completing all documentation and billing in a timely manner.
Hours:
This is a part time weekend position. Hours are Sat and Sun 9-5pm (flexible on timing).
This role is about making a difference in the lives of the people we serve. As a Care Coordinator, you'll bring the following qualifications:
- A Bachelor's Degree in psychology, social work, health care, administration, or related human service field is desirable but not required
- Minimum of two years' experience with social service organizations
- Experience with coordination and referral for services is preferred
- Excellent communication skills, organization skills, and computer skills are required
