The Judge Group is currently seeking RN Case Managers for a client of ours in Orange, CA!!
Schedule: M-F 40hrs/week
Position Overview:
The Medical Case Manager will be responsible for providing intensive case management, including assessment, planning, implementation, coordination, monitoring, and evaluation of our members’ needs. This role is crucial in promoting quality and cost-effective outcomes for all members.
Duties & Responsibilities:
- Assessing member needs using standardized health needs or risk assessments.
- Performing comprehensive, disease-specific clinical assessments.
- Developing and implementing care plans with SMART goals.
- Coordinating care and services with members, family members, and other providers.
- Facilitating referrals to behavioral health/substance use disorder services and community resources.
- Participating in Interdisciplinary Team meetings and collaborating with interdepartmental staff.
- Advocating for members’ best interests regarding funding, treatment alternatives, and coordination of care.
Qualifications:
Minimum Qualifications:
- Associate degree in nursing (ADN) or related field required PLUS 3 years of clinical experience required and/or managed care experience required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
Preferred Qualifications (Not Required):
- Bachelor of Science in Nursing (BSN) degree or related field.
- Case Management Certification (CCM).
- Bilingual in English and one of the clients Health’s defined threshold languages (Arabic, Chinese, Farsi, Korean, Spanish, Vietnamese).
