We are seeking a friendly and compassionate Primary Care Nurse Practitioner to join our team. This role involves providing comprehensive care to patients in long-term care facilities.
Responsibilities
Provide comprehensive healthcare services and coordinate patient care Work with a team of healthcare professionals Assess, diagnose, and treat residents' physical and mental well-being Conduct patient assessments, including medical history and physical exams Order and interpret diagnostic tests Develop diagnoses and treatment plans Prescribe medications and administer treatments Provide counseling to residents and families Collaborate with the interdisciplinary team for seamless care Manage chronic conditions and monitor progress Adjust care plans as needed Commit to person-centered care Create a safe and nurturing environment for elderly residents
Requirements
Nurse Practitioner License DEA License 6 months experience in long-term care
