Requirements:
- Active MI license
- Submit rates at the time of name clear
- Covid vaccinated
- BLS/ACLS certified
- Board Certified
- Notate participation in MI Medicaid/CHAMPS at time of name clear
- Clean background with no disciplinary actions and no malpractice
- DEA - Verbal Confirmation Required at Name Clear
