Role Description
This is a full-time hybrid role for a Hospice Licensed Practical Nurse at Gracious Care Hospice targeting the Salem, OR area and its surroundings, with flexibility for some remote work. The Licensed Practical Nurse will be responsible for providing compassionate and skilled nursing care to end-of-life patients in different settings: home, assisted living facilities, adult care homes, memory care facility, nursing homes, etc. The role involves ensuring patient comfort, managing symptoms, and supporting families during challenging times.
Some of the tasks include but are not limited to:
- Conduct physical examinations and assessments of patients
- Assist with medication administration education and treatments as prescribed by physicians
- Monitor and document patient vital signs and medical information
- Assist with activities of daily living, including bathing, dressing, and grooming
- Provide specialized care for patients with memory impairments in a memory care unit
- Perform airway management, including suctioning and oxygen administration
- Provide catheter care and assist with urinary catheterization procedures
- Educate patients and their families on health coaching techniques for managing chronic conditions
Qualifications
- Licensed Practical Nurse (LPN) with active license in Oregon
- Experience in nursing, particularly in end-of-life care, home health and/or hospice.
- Familiarity with home health and hospice regulations and standards
- Knowledge of anatomy, physiology, and medical terminology
- Ability to communicate effectively and empathetically with patients and their families.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Excellent observation, documentation, and organizational skills
- Ability to work independently and as part of a multidisciplinary team.
- Experience working with veterans and families is preferred.
- Excellent problem-solving skills to address patient needs and emergencies promptly.
