About the Role:
Job Summary
Under the general direction of the Practice Manager, coordinates and manages access including all urgent and semi-urgent patient referrals to and from the practice and other ancillary programs in all NGHS and surrounding markets involved in the course of patient care. To facilitate the process a Hepato-Pancreato-Biliary patient will go through as a patient of NGHS evaluating systems-based practices and optimization of existing resources. Program Coordinator will serve as primary clinical contact as well as be responsible for HPB-NSQIP database management, including development of scheduled and requested reports. Evaluates ongoing systems processes and develops quality improvement processes to enhance clinical care and resource allocation.
Minimum Job Qualifications
- Licensure or other certifications: LPN
- Educational Requirements: Associates degree required. Four (4) additional years of clinical experience in hospital or clinical practice setting might be considered in lieu of degree.

Essential Tasks And Responsibilities
- Cultivates strong relationships with top strategic referrers to increase access and case volume.
- Serves as first point of contact to patients, their families/representatives, outside agencies, physicians, and referrers. Responds to inquiries for access to Hepato-Pancreato-Biliary care, inquiries typically received through Epic work que, Online Inquiry
- Responsibilities for coordination of outpatient referrals and when necessary coordinating admission/scheduling for appropriate inpatient needs for NGPG General Surgery and Hepato-Pancreato-Biliary services.
- Actively markets NGPG Hepato-Pancreato-Biliary treatment types to internal clinics and external groups through direct education to clinic staff, health fairs, community outreach fairs, and/or community events.
- Maintains and develops working relationships with referral coordinators, both internal and external, to ensure no barriers or delays in scheduling of consultations to the clinic.
- Evaluates and develops improved practices for all clinical care of the Hepato-Pancreato-Biliary program. Serves in a primary nursing role working closely with the surgeon and PA during clinic as well as non-clinic days as needed.
- Coordinates with physician leadership and designees (i.e. resident physicians) in Hepato-Pancreato-Biliary Surgery to review and develop quality improvement processes within the program. Coordinates with NGHS and NGMC leadership as needed for quality improvement within the scope of the program
- Responsible for developing and evaluating data reports from the HPB-NSQIP database system.
- Works closely with surgery scheduler in order to optimize all surgical scheduling and patient management.
- Coordinates with administration of NGHS, NGPG, and Cancer Center in program development and quality improvement.
- Identifies trends with high volume referring offices and works closely with Business Development to continually develop positive community referral relationships.
- Maintain Qgenda schedules for Surgeon and Resident Call Schedules as requested.
- Order Supplies, Identify and enter IT ServiceNow Tickets, Report variances using RL6 system software.
- Schedule Surgeries/Procedures as needed.
- Other duties as assigned.
