Responsibilities
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive community health nursing services to pregnant and postpartum individuals and infants until the baby turns two years old. Nurses maintain the highest standards in clinical nursing practice, adhering to the NFP model and to policies, procedures, protocols, guidelines, and standards of NFP and of The Foundation for Delaware County.
The Nurse Home Visitor provides screenings, education, support, and referral resources and assists women and teenagers in attaining their targeted goals. A caseload consists of 25 clients or more. The Nurse Home Visitor completes all required education, including training with the NFP National Service Office in Denver, Colorado. Charting and data entry are completed within the appropriate timeframe. Weekly NFP team meetings, case conferencing and reflective supervision are required. The position is full-time.
Qualifications
•BSN required.
•Current RN (PA) license (in good standing) required, maintains licensure.
•Current CPR certification required.
•Current child abuse clearance required.
•Two years recent experience in maternal/child health, substance abuse treatment, public health, or mental/behavioral health nursing preferred.
•Home visiting experience preferred.
•Excellent written and verbal communication skills required.
•Basic computer and data entry skills required.
•Will need to use your own reliable transportation to travel across the county and region to business-related activities and events. Mileage reimbursement is available at the approved IRS mileage rate.
•Must be able to work a flexible schedule including some evenings and weekends.
