Hourly Pay Range (Minimum - Midpoint - Maximum):$50.611 - $65.305 - $79.999
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will DoAssists patients, staff and physicians as needed to maintain a high level of efficiency of operations and customer service while providing quality care to the designated patient population. Assists with the Leadership of the Nursing and Patient Service Representative staff. Serves as technical nursing expert for assigned areas. Under the supervision of the Nurse Practitioner, provides a higher level of care specializing in the assessment and management of chronic wounds, ostomies and issues with continence.
Required Qualifications- 3 Years RN experience in related clinical area.
- 1 Year Wound Care experience.
- California Registered Nurse (RN) - CA Board of Registered Nursing -REQUIRED
- Certified Wound Ostomy Nurse (CWON) - The Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB) -REQUIRED
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED

Essential Functions - Clinical skills

Able to prepare patient for examination or procedure. Obtains complete and accurate consent for procedures/surgery. Prepares and labels biopsies in a thorough and accurate manner.
Accurately documents patient care per protocol.
Partners with physicians to continuously learn and expand clinical knowledge base.
Listens to patients. Collects pertinent information, recognized the urgency of the patient's problem and routes to the physicians. Returns phone calls according to physician instruction. May schedule patients' appointments.
In partnership with Physicians, provides specific educational material and individual teaching.
Uses universal precautions and demonstrates knowledge of infection control policies and procedures.
Department specific requirements: all in collaboration with the Nurse Practitioner/referring provider:
Wound and Skin Care:
Management of Chronic, Surgical, Diabetic, Venous, Arterial and Traumatic wounds. Simple to complex wound dressing management. Compression wrap application. Compression sticking application. Cast management.
The CWOCN performs routine patient assessment regarding condition changes; signs and symptoms of infection; worsening wound; new wound; skin changes to peri-wound; abnormal vital signs; swelling issues; exudate changes; increase in pain; etc. with a timely report to provider.
Comprehensive assessment and directing the plan of care: history and physical examination; wound assessment; comorbidity assessment; dietary assessment; risk and prevention assessment.
Management of chronic wounds (e.g. arterial/venous/neuropathic complications, surgical wounds, tumors, pressure ulcers, etc.).
Implementation of appropriate treatment modalities: compression therapy Negative Wound Therapy (NPWT) simple dressing changes to ensure a moist wound healing environment, pressure reduction methods, management of moisture associated skin damage (MASD), total contact casting, palliative care, etc.
Referrals to other specialties: Vascular Surgery, General Surgery, Plastic Surgery, Orthopedics, Lymphedema clinic, Physical Therapy, etc.
Debridement techniques: sharp, enzymatic and chemical.
Ostomy Care:
Comprehensive assessment and directing the plan of care: history and physical examinations; preoperative site marking; postoperative stomal and peristomal skin assessment with routine follow-up; comorbidity assessment; dietary assessment; risk and prevention assessment; life style management; sexual counseling.
Provides routine care that promotes optimal outcomes for the ostomy patient (to include: assessment of the stoma, peristomal skin, stomal output, measurement and fitting/sizing of the pouching system).
Provides and/or assist with individualized preoperative and postoperative education regarding basic ostomy care to patients.
Families, caregivers and Team members.
Continence Care:
Comprehensive assessment and directing the plan or care: history and physical examination; comorbidity assessment; dietary assessment; risk and prevention assessment: lifestyle management.
Implementations of appropriate treatment modalities: management of moisture-associated skin damage (MASD), bowel/bladder regimes/diaries, self-catheterization, constipation/diarrhea management, dietary modifications.
Referrals to other specialties: General Surgery, Urology, etc.
Teaching: The CWOCN also provides excellent patient and family education on wound care management, lifestyle changes and holistic patient care. Compression Therapy Education, etc. in accordance to the Wound Clinic guidelines.
Collaborates with the supervising Wound Clinic Nurse Practitioner or Physician, and in accordance with established protocols. - Department efficiency and effectiveness
Organizes and completes daily requirements and responsibilities.
Telephone Message Management - Ensures prompt and efficient return of messages according to established policy.
Troubleshoots and resolves problem calls.
Daily organization - Monitors patients flows, physician schedules and complete daily tasks to ensure quality and meet service standards. Follows policy and procedure for entering of OCM. Completes work within assigned hours.
Responds to changing circumstances and prioritize patient needs.
Responds to urgent and emergent situations in a calm and capable manner. Utilizes good judgment and problem solving ability.
Ensure readiness of reports and information to maximize patient visit.
Maintains appropriate supply levels. Cost conscientious in usage.
Completes department inventories per guidelines.
Participates in Quality Improvement Programs. - Leadership
Works collaboratively with Wound Clinic Nurse Practitioner to ensure smooth day-to-day operations within respective Team, ensure the care provided to patients' meets regulatory and organization standards, quality patient care and effective and efficient utilization of resources.
Provides Leadership to staff to ensure multidisciplinary communication, collaborative working relationships and a patient/customer-oriented environment responsive to the needs of patients.
Participate in orientation of new staff, acting as a resource and identifying opportunities for staff development for new and existing Team members.
Works collaboratively with the manager to facilitate the provision of quality care.
Works collaboratively with the Supply Chain Services Department in relation to annual review of wound formulary and with wound ostomy supply vendors, aiming for quality cost effective outcomes.
Works collaboratively with various supply/distribution vendors and DMR providers and remain current with newest dressings/products.
Mentors and encourages the current and future WCC staff to enhance the expertise throughout the clinical operations to benefit patient outcomes.
Assists with training and education of new and existing staff.
Consistently pro-active in team development and in problem-solving to meet department goals and support changes.
Acts as a resource and role model for staff.
Facilitates inter-departmental cooperation and teamwork.
Adheres to Sharp Healthcare standards of conduct. - Medication management
Maintains current knowledge of applicable medications.
Demonstrates safe and accurate administration techniques. No medication errors. - Nursing Competencies
Successfully completes Nursing Competencies with a score of 90% or greater in each section = Great.
Successfully completes Nursing Competencies with a score of 90% or greater on all but one section. Passed on retesting = Good.
Unsuccessful in passing more than one section of Nursing Competency Testing and/or failed to pass testing on second attempt = Needs Improvement. - Quality initiatives
Completes Quality Assurance (QA) assignments and ensures overall department compliance with requirements. QA scores: 96- 100%=Great; 91-95%=Good; 90% and below=Needs Improvement.
Support and completion of quality initiatives: P4P, BMI, Date Mart, etc. - Technical skills
Demonstrates knowledge of equipment and information systems applications.
When necessary, activate emergency procedure per protocol - code, fire, etc.
