Baylor Scott & White Hillcrest - Cardiology Nurse Navigator -The Cardiac Nurse Navigator is a Registered Nurse who supports the cardiology teams by promoting evidence-based practice and care coordination for cardiac and cardiothoracic patients in a multidisciplinary approach. Responsibilities include but are not limited to providing evaluations for patients as they enter the cardiac program, assist in establishing plans of care, provide patient education pre and post operatively, and collaborate with inpatient and outpatient care teams to achieve optimum care and outcomes.

Minimum Requirements Graduate of RN School of Nursing

Minimum of 6 Months of RN experience
