What Will You Be Doing
We are looking for an experienced, talented, and compassionate
Intake RN or LPN Baylor to work 24 hours/week and get paid for 32 hours/week. In this position you would work 3 days per week, Friday, Saturday and Sunday, 8 hours per day and is located in the
South Shore area.
You will be responsible for overseeing the appropriateness of referrals to the Agency’s Intake Department and ensuring that the processing of referrals for home care services is completed on a timely basis. You will work with Clinical Managers in assisting to coordinate the Agency’s daily workload and appropriateness of patients for agency services. You will build relationships, educate and support referral sources.
What We’re Looking For In You
- 3+ years of nursing experience
- 1+ year in direct home health care preferred.
- Works with Intake CTAs to oversee process new referrals.
- Strong clinical assessment skills and the ability to prioritize and triage patients.
- Exceptional communication skills and the ability to work within a team structure.
- Excellent organizational skills and time management skills.
- Build relationships, educate and support referral sources.
- Current Massachusetts License (in good standing).
- Medicare/Managed Care experience required.
- Familiar with medical data base programs (i.e. Health Wyse, etc.).
