Circare is a not-for-profit community based mental health agency located in Syracuse, New York. Circare programs are designed to help individuals and families, who have encountered certain obstacles, develop the resources and supports to live a satisfying, naturally independent life.
The ACT Psychiatric Nurse Practitioner will serve our patient population in our Assertive Community Treatment program. This is a Full-Time position working Monday through Friday, 8:00am-4:30pm.
Principle Duties and Responsibilities:
· Conduct psychiatric assessments, including diagnosis, history, course of illness, and response to treatment, mental status exam
· Provide ongoing psychiatric care to clients, including evaluation and assessment, diagnosis, and medication monitoring
· Explain role of NP and medications in mental health treatment and recovery
· Work in collaboration with the ACT Medical Team, including Psychiatrist and RN
· Provide crisis assessment and management of clients, both on-site and over the phone
· May provide wellness activities as needed such as health monitoring, consultation with other healthcare providers, or directing nursing staff to monitor physical health circumstances and conditions
· Communicate and consult with teammates to shape quality practice
· Assist the ACT Team Leader/Clinic Director in training staff in knowledge and skills basic to treatment of persons with severe and persistent mental illness
Job Requirements:
· NYS licensed/registered NP with specialty area of Psychiatry required
· Must have NPI number
· National Certification through AANP or ANCC
· Master's of Science in Nursing
· 2 years of experience providing NP services to adults in mental health settings; preferably in OMH licensed facility
· Knowledge of evidence-based practices and/or person-centered care a plus
· Must have a valid driver’s license and own transportation
· Maintain individual Medical Malpractice insurance coverage of at least $1 million dollars, Circare will reimburse NP for the cost of the premiums annually.
· Circare’s insurance carrier requires that Circare employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date on Declaration Page.
