Role Description
This is a full-time on-site role for a Case Management Nurse at Takara Hospice in San Diego, CA. The Case Management Nurse will be responsible for discharge planning, utilization management, nursing care, and medical case management on a day-to-day basis, ensuring the highest quality of care for patients nearing the end of life.
Qualifications
- Nursing and Medicine skills
- Case Management and Utilization Management expertise
- Experience in discharge planning
- Compassionate and empathetic approach to patient care
- Strong communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Active RN license in the state of California
- Certification in Hospice and Palliative Care Nursing is a plus
