Job Summary
Our client is seeking Licensed Social Workers (LSWs) and Registered Nurses (RN's) who are dedicated and compassionate, and who appreciate an excellent work/life balance. Care Managers will provide intake, assessment, and care management services to clients, ensuring their needs are met through a collaborative process.
Job Responsibilities: Provide support/education to clients with complex social/behavioral/environmental issues. Conduct home visits. Utilize clinical judgment and critical thinking skills to facilitate appropriate client physical and behavioral healthcare. Develop, assess, and adjust patient-centered care plans. Monitor the effectiveness of care plans and establish measurable goals and outcomes. Provide coaching, information, and support to empower clients. Report specific health and social information to the integrated team. Conduct advanced care planning. Utilize Service Guidelines for developing service plans/care plans. Authorize services efficiently to meet client needs.
Qualifications: Ohio licensed Social Worker or Registered Nurse in good standing. One year of experience with persons with disabilities/chronic conditions and LTSS. Experience with mental/behavioral health populations. Experience with electronic documentation. Prior case management experience. Knowledge of issues related to the elderly population. Strong critical thinking skills and clinical judgment. Ability to learn in-person, through written formats, and virtually. Above-average technology skills. Ability to work in multiple software systems. Knowledge of community resources in Southwestern Ohio. Good interpersonal communication skills. Adaptability to frequent change. Valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton, and Warren Counties. Proficient in MS Word, MS Excel, data entry, and learning new software.
