The person in this position will provide personal care and related services according to an established care plan to terminally ill patients to augment the care given by family members or others in the home, nursing home, assisted living residence, or hospital.
QUALIFICATIONS:
- Must have NJ Home Health Aide Certification
- Ability to travel within the state of New Jersey
- Meet physical requirements: bending, pushing/pulling, standing, lifting up to 50 lbs
RESPONSIBILITIES (including, but not limited to):
- Follow Aide Care Plan. Document all activities using electronic software
- Assist the patient to maintain good personal hygiene, assisting with bathing, skin care, hair, nails, teeth, and mouth, as well aschanging bed linens per the care plan
- Help patients get in and out of bed and assist them to ambulate if required by care plan
- Assist patients with the use of durable medical equipment as per the care plan
- Report to the Grace Registered Nurse any change in the patient’s medical, nursing, and social needs as related to the plan of care
- Report to the Grace RN any challenges or concerns regarding a patient’s home/facility environment
- Plan and prepare nutritious meals in accordance with dietary restrictions and assist in feeding patients when necessary as per the care plan
- Maintain a healthy, safe, and clean environment for the patient
- Perform homemaking duties as assigned
- Assist patients with prescribed exercises as required by the care plan
- Maintain up-to-update clinical knowledge by attending Grace’s mandated educational program(s)
- Attend mandatory quarterly aide meetings and other meetings
- Maintain and complete appropriate documentation records daily using electronic software according to Hospice, State, and Federal requirements
- Work according to scheduled hours
- Adhere to the practice of confidentiality with regard to patients, staff, and customers
