What We Are Looking For
The Behavioral Health Specialist, LCSW in coordination with all primary care providers including physicians, nurse practitioners, physician assistants and substance abuse counselors to deliver integrated care services. This position also sees clients in the outpatient setting. The Behavioral Health Specialist, LCSW provider, is responsible for providing brief therapy behavioral health interventions to SAC Health medical clinic patients. The focus of the Behavioral Health Specialist, LCSW in primary care is to conduct assessments/screenings and provide therapeutic interventions where needed. In this setting, there is significant attention to psychosocial stressors that may impact compliance with medical treatment and prevention activities that may prevent a need for referral to specialty behavioral health services. In addition, will develop treatment plans that will be integrated with the patient's medical treatment plan (in compliance with all HIPAA and Privacy guidelines) and will work closely with the medical team to meet the needs of individual patients. Works closely with the Health Care Team to coordinate case management activities and coordinate supervision of students who rotate through SAC Health Behavioral Health as part of their training through
coordination with clinical supervisors.

ESSENTIAL FUNCTIONS AND DELIVERABLES
Ability to use behavioral health intervention as part of an integrated care response to patients with chronic health needs, mental health needs, or who may be at risk for mental health-related issues. Uses techniques to improve behavioral compliance in those with chronic medical conditions and provides evidence-based treatment using a whole health approach.
Provides therapy to SAC Health patients in Primary Care and the Outpatient setting. May serve as the patient's primary contact for the primary care team.
Respond and initiate care to patients who are in primary care rooms as part of a wholeness approach to health care. Develops and tracks the patient's progress in the shared care plan.
Responds to provider requests for behavioral health interventions in the exam rooms.
Provide therapeutic interventions when behavioral health issues arise while medical treatment is being provided. Provides brief interventions and assesses whether additional interventions will be required and may schedule follow-up in primary care or make a
referral to the specialty clinic based on the needs of the client. For those who will benefit from brief interventions, the preference is for treatment in the primary care setting.
Develop diagnosis and treatment plans that are integrated with the medical treatment planning in Primary Care teams. Supports patient self-management skills through education, health teaching, resources, motivational support, engagement, and peer utilization.
Regularly attend primary care team meetings to evaluate the progress of patients.
Complete all required clinical and billing documentation via the EHR records.
Represent the agency to other groups and outside organizations as assigned.
May supervise graduate students/interns as assigned.
Participate in utilization review activities as assigned.
Tracks medical and behavioral health outcome measures in the patient EMR. Coordinates care with other team members of the care team.

QUALIFICATIONS:
Education: Must possess a Master's Degree in Social Work (MSW).
Licensure/Certification: Must be eligible for LCSW licensure or PhD of PsyD within three (3) years of date of hire. Valid CA Driver's License. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
Experience: Minimum of 1-2 years of licensed clinical experience is preferred.
Essential Technical/Motor Skills: The ability to use a computer on a routine basis and complete required documentation. Use of word processor, spread sheets and internet. Ability to transition to Electronic Health Records.
Interpersonal Skills: The ability to communicate positively and effectively with all staff in a multidisciplinary environment. Ability to conduct meetings and present in a public forum. Strong interpersonal relations skills. Fluency in English and Spanish preferred. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team environment.
