Summary:
The Nurse Program Coordinator is responsible for assessment, coordination of care, discharge planning, nursing care delivery, evaluation, and documentation of care for pediatric patients and their families.
Responsibilities:
1. Assesses patient/families physical, emotional, spiritual, psychosocial and learning needs; communicates pertinent information to the team and other health care providers; assess and recognize changes in patient condition and family dynamics.
2. Provides care coordination and care planning for target population
3. Provides nursing care to target population of patients and documents evaluation and care delivery in patient plan of care.
4. Determines the education needs of nursing personnel, multi-disciplinary team members and others; implements patient and family education in collaboration with the multi-disciplinary team.
5. Provides leadership to the nursing team and acts as a change agent to improve patient care.
6. Serves as a resource to other nurses and healthcare providers with expertise in assigned practice area.
7. Other duties as required.
Other information:
Technical Expertise
1. Experience in pediatrics is required
2. Experience in communication, teaching skills, and problem-solving skills is required
3. Experience working with all levels within an organization is preferred
4. Experience in healthcare is required
5. Proficiency in MS Office Outlook, Excel, Word or similar software is preferred. Epic software is preferred.
Education and Experience
1. Education: Bachelor's degree is required
2. Certification: RN Licensure is required
3. Years of relevant experience: 3 - 5 years is required
4. Years of experience supervising: None.
