As a Family Nurse Practitioner at Vivia Health, you will have a direct impact on the health and wellbeing of a diverse adult patient population in underserved communities. You will collaborate with an interdisciplinary team on preventative care, disease management, and overall patient wellness.
What will your role entail?
- Provide exceptional primary and preventative care to all patients
- Conduct comprehensive health assessments, including physical examinations and patient histories; Diagnose and treat acute and chronic illnesses, conditions, and injuries; Chronic disease health care management and education
- Perform medication reconciliation to refill or prescribe appropriate medication regimen based on assessment and diagnosis
- Order, perform, and interpret the results of diagnostic tests and lab work (e.g., blood tests, X-rays, EKGs)
- Conduct minor procedures including suturing, wound care, incision, drainage etc.
- Educate patients and their families on health management, disease prevention, and healthy lifestyle choices
- Collaborate with physicians, specialists, and other healthcare professionals to ensure comprehensive patient care
- Educate patients on their physical and mental health conditions, treatments, and care plans
- Maintain accurate and up-to-date patient medical records
- Document patient interactions, diagnoses, treatments, and outcomes in electronic health records (EHR)
- Participate in practice management and quality improvement initiatives
- Assist in the development and implementation of clinic policies and procedures
- Supervise and mentor nursing staff, students, and interns
What do you bring as a Clinician?
- Master of Science in Nursing, or Doctor of Nursing Practice; 1-2 years of experience as a nurse practitioner in primary care
- National certification as a Nurse Practitioner through AANP or ANCC
- Licensed to practice in Illinois with current Illinois registration and in good standing.
- Active control substance and DEA license preferred
- Full Practice Authority
- Record of prior stable employment and/or favorable references.
- Experience or exposure with mental health and substance use disorders
- Strong collaboration and teamwork
- Excellent communication and interpersonal skills
- Proficiency in using EHR systems and other healthcare software
- Strong organizational skills
- Attention to detail
