Position Overview: We are seeking a compassionate and experienced Family Nurse Practitioner (FNP) to join our team. The FNP will provide advanced nursing care to patients of all ages in their homes, collaborating with physicians and other healthcare professionals to ensure the highest standards of patient care. This role requires a blend of clinical expertise, excellent communication skills, and a patient-centered approach.
Key Responsibilities:
- Conduct comprehensive health assessments, including patient histories and physical examinations.
- Diagnose and treat acute and chronic illnesses, injuries, and conditions.
- Develop, implement, and manage individualized treatment plans.
- Prescribe medications and other treatments in accordance with state and federal regulations.
- Provide preventive care, including immunizations, screenings, and health education.
- Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families about health conditions, treatments, and preventive care.
- Maintain accurate and up-to-date patient records and documentation.
- Participate in quality improvement initiatives and ongoing professional development.
Qualifications:
- Master’s or Doctoral degree in Nursing from an accredited program.
- Current and valid Nurse Practitioner (NP) license in the state of Michigan.
- Certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
- Minimum of 2 years of clinical experience as an FNP, preferably in a home health or primary care setting.
- Excellent diagnostic and clinical skills.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Valid driver's license and reliable transportation for home visits.
