Site practices a Patient Centered Health Home and all employees are an integral part of this model of care delivery. Providers enjoy a lower patient volume with no call, no weekends, no inpatient, no after hours required!
This multi-site health program provides high quality, culturally competent outpatient care to an underserved patient population, so a rewarding chance to impact the health of a community. Comprehensive services including medical, dental, pharmacy, behavioral health, lab and other community health and outreach programs.
