Role Description
This is a full-time on-site role for an RN Case Manager at OneWell Health Care. The RN Case Manager will be responsible for tasks such as discharge planning, utilization management, nursing care, medicine administration, and case management to ensure comprehensive and personalized care for our clients.

Case
- Management skills
- Nursing and Medicine knowledge
- Strong organizational and communication skills
- Ability to work collaboratively in a team setting
- Prior experience in case management or similar roles
- RN license in the state of New Jersey
- Knowledge of Intellectual and Developmental Disabilities care is a plus
- Bachelor's degree in Nursing or related field
