We are seeking a skilled Registered Nurse that will be a Nurse Case Manager responsible for improving a patient’s quality of life by planning, directing, and providing home care services. This licensed nursing professional will provide direct nursing care and manage a team of therapists and other professionals to treat patients/clients under a physician’s plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Nurse Practice Act, and regulatory entities. The nurse case manager’s goals are to rehabilitate and educate the patient to regain and retain independence in their home setting. The provided care plan will include all clinical care, symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.

Key Duties Include:
· Follow Agency Policies and Procedures, Federal and State Regulations and Standards of Practice.
· Attend Agency In-services, Case Conference, or other meetings as requested.
· Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s home.
· Be responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. (See Tactical Deliverables)
· Records the health needs of the patient.
· Performs and completes start-of-care assessment, medication profile, initiates Nursing or Home Health Aide plan of care, and/or Homemaker/Companion service plan and associated paperwork in accordance to agency policy.
· Evaluates the effects of the care given and regularly reevaluates the patient’s nursing needs.
· Evaluates and revises the plan of care, as necessary.
· Provides those services requiring substantial specialized nursing skill based on the physician’s orders and the nursing diagnosis. Examples of such services include, but are not limited to; physical assessment, patient and family teaching, urinary catheter insertion.
· Foster a positive relationship with patients/caregivers and doctors to improve patient outcomes.

Education and Experience:
· Currently licensed as a Registered Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.
