Compensation: $80,000 - $130,000 per year + $6000 in Bonuses During First Year of Employment!

RN Case Manager Overview:
As a valued member of our team, you'll:
- Screen and assess clients, develop and implement patient care plans, and monitor care delivery to ensure safe, medically appropriate, and cost-effective care.
- Complete OASIS timepoint visits, facilitate referrals, and coordinate care transitions between settings.
- Communicate and collaborate with the patient's primary provider, healthcare and community providers, and other members of the interdisciplinary care team.
- Provide patient/family education and counseling, assess psychological, psychosocial, and psycho-spiritual needs, and ensure compliance with organizational and regulatory standards.
- Work with our multidisciplinary team to provide quality care tailored to each patient's physical and emotional needs.
Qualifications:
To excel in this role, you'll need:
- A current Registered Nurse (RN) license in the state of Minnesota, along with at least three years of nursing experience (home care experience preferred).
- Knowledge of home care regulations, standards, and quality indicators, strong organizational, assessment, and problem-solving skills, and proficiency in computer systems and software applicable to job duties.
- A valid Driver's License and reliable vehicle with current insurance.
