Responsibilities:
- Community outreach and education to develop home care referrals
- Provide support and guidance to clients, family members and caregivers
- Develop and implement home care plans for clients
- Perform Supervisory Care Assessments
- Monitor client progress and make necessary adjustments to care plans
- Coordinate with other healthcare professionals, such as doctors, nurses, physical therapists, and social workers, to ensure clients receive the best possible care
- Maintain accurate and up-to-date records of client care plans and progress
- Ensure compliance with all applicable regulations and laws
- Caregiver training as needed

Qualifications:
- Must have Active LPN/RN license in Georgia
- Minimum of 3 years of home care experience, with at least 1 year in a supervisory role
- Strong clinical skills and knowledge of home care practices
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic attitude toward clients and their families.
- Reliable transportation and valid driver's license.
- Clear background check and drug screening.
