Additional Details:
Inpatient weekend census for general surgery patients will vary from around 10 to 25. On a really busy weekend, it could be 30. That does not happen very often.
ER will average about 0-3 admissions per night after hours. Sometimes you can just admit patient over the phone and see in the morning. It's not common to have to operate in the middle of the night, but it does happen.
On an average weekend, you'll see 0 and 6 cases. Most often it will be 2-4.
Sometimes the cases are small like I&Ds but sometimes you get a big abdominal case (perforated ulcer, colon, GI bleed).
Busy hospitalist service will usually consult a couple of times a day.
Have most medical sub-specialties including Cardiology, Pulm/Critical Care, and GI. We have orthopedics. No Neurosurgery.
Case Complexity:
Practice does most abdominal surgery other than liver and pancreas surgery.
Rare chest case (empyemas mostly, not lung resections).
Hospital performs thyroid, parathyroid, breast, pretty much all aspects of general surgery.
Little pediatrics beyond abscess drainage.
Hospital sees most trauma. If it is a complex trauma patient that is stable, sometimes will ship to trauma center. No neurosurgeon, so trauma of the head gets shipped out without being involved, unless there is some coexisting abdominal trauma.
Most common cases seen in ER are abscesses that need draining, appendectomy, incarcerated hernia, small bowel obstructions, cholecystitis/gallbladder problems, perforated ulcer, perforated diverticulitis. Typical bread and butter general surgery emergencies.
