Job Title: Home Health Registered Nurse (RN)
Location: Burlington, Camden, and Gloucester County
Job Type: Full-time (Monday to Friday, Day Shift, 40 hours per week)
Overview:
We are currently seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team. The ideal candidate will have experience in home health care, particularly with OASIS assessments, and will play a vital role in providing quality patient care in the comfort of their homes.
Qualifications:
- Current RN license in NJ
- Experience with OASIS assessments
- Previous background as a Certified Homecare RN or LPN is preferred
- Hospital nursing experience is a plus
Key Responsibilities:
- Assess, diagnose, plan, implement, and evaluate patient care based on outcome identification and evidence-based practices.
- Coordinate patient care and communicate extensively with other members of the healthcare team, patients, families, and significant others to develop and implement an effective plan of care.
- Supervise Home Health Aides in the home setting.
- Promote the delivery of quality care while efficiently utilizing agency resources.
- Conduct home visits to 5-6 patients daily, performing comprehensive assessments that incorporate psychosocial and physiological status, health behaviors, and compliance with treatment regimens.
- Coordinate treatment plans with physicians through timely and effective communication.
- Administer, monitor, and document therapeutic interventions and treatment regimens.
- Maintain knowledge of documentation standards, point-of-service devices, and completion time frames.
- Educate patients and their families about disease processes, individual care needs, wellness, medications, and safety concerns.
- Collaborate with the healthcare team to establish and modify the plan of care as necessary.
- Adhere to standard precautions and principles of patient safety.
- Identify and participate in performance improvement initiatives.
- Maintain professional and departmental competencies.
