Krucial is hiring for a Case Manager, RN contract opportunity. This candidate is responsible for reviewing and analyzing patient admission information in line with established policies using case management software. The role involves assessing patients' physical, psychosocial, cultural, and spiritual needs through observations, interviews, and interactions with patients, physicians, and interdisciplinary teams. This position facilitates decision-making for the next level of care, coordinates discharge planning, and ensures accurate documentation in the electronic medical record (EMR). The Case Manager will also handle utilization management tasks, participate in performance improvement activities, and collaborate closely with Social Workers, Homecare Coordinators, and other care team members to ensure timely and efficient service delivery. Key responsibilities include managing utilization reviews, tracking length of stay, and working with third-party payers for approval of patient stays. The role requires applying evidence-based criteria and regulatory guidelines to ensure appropriate care and compliance. Floating may be required to any Common Spirit location within sixty (60) miles of the original assignment location.
Must have a current Registered Nurse CO State or compact license, at least 2-years experience as a Case Manager RN, BLS, and discharge planning experience. Associate degree in nursing is required, bachelor degree in nursing is preferred.
