Great opportunity in the Los Angeles and Orange County areas of California for a Nurse Practitioner to perform annual wellness exams working for a private group.
- Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
- 2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
- Perform an annual wellness exam for senior Medicare patients
- Collaborate with Primary Care team to ensure completion of holistic patient care
- Combination of home health and telehealth - 30 mile radius
- If you are not located in East Los Angeles you must be comfortable commuting to the East Los Angeles area and then an additional 30+ miles within the East Los Angeles area
- Must have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.
- Mon-Fri 8a-5p
- 8-10 patients per day
