Transition Specialist role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. If you are in Travis service area or in the Austin, TX, area, you will have the unique opportunity to support members through the Texas Promoting Independence Initiative move from institutional into the community.
Field based position with home-based office. Travel up to 75%
Primary Responsibilities: - Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive community-based setting
- Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
- Provide pilot services including Cognitive Adaptation Training (CAT), other intensive psychosocial rehabilitation and transition services
- Complete specified screening instruments on each member every six months in the program
- Communicate and coordinate with member's assigned service coordinator twice monthly to ensure continuity of care
- Participate in all training activities and learning community with all stakeholders (HHS, third party evaluators, members, etc.)
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
- Make outbound calls and receive inbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Assist with providing housing option and collecting documents
- Conduct in person visits/assessments

Required Qualifications: - Bachelor's Degree in health, social services or a related field (or higher) AND 1+ years of relevant experience in assisting people in transitioning from institutional settings to the community
- 3+ years of experience working within the community health setting in a health care role
- 1+ years of experience working with persons with a serious mental illness (SMI) and/or experience of mental illness
- The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices
Preferred Qualifications: - Master-level Social Worker (LMSW, LCSW) or Professional Counselor, Licensed Bachelor Social Worker (LBSW), Provisionally Licensed Psychologist (PLP), or Licensed Psychological Associate (LPA) licensed to practice in Texas
- A background in managing populations with complex medical or behavioral needs
- 5+ years of experience working with MS Word, Excel, and Outlook
- Case management experience
- 5+ years of experience documenting in an electronic documentation system
- Certified Case Manager (CCM)
- Experience in discharge planning
- Experience with arranging community resources
