ABOUT THE ROLE:
The focus of this position is to ensure the efficient delivery of quality case management and nursing services to Tepeyac’s patients. Case Management ensures a client, and their caregivers are central to the role in coordinating and managing their care. The RN/CM lead will collaborate closely with the primary care provider, assuring comprehensiveness of care across disciplines and engaging external partners in coordinating care, as necessary. The RN/CM lead will strive to provide access to quality care regardless of payor source while reducing healthcare costs. The Case Management will coordinate population health management initiatives through the supervision of the registered nurse and case manager teams and provide direct clinical care and medical case management services to patients. This clinic position facilitates patient care throughout the entire visit and manages patient flow on the care team while contributing to continuity of care to improve patient and quality-improvement outcomes. The Case Management provides staff training, assists in development and monitoring of reports, and manages projects in support of quality, risk, and compliance program goals.
RESPONSIBILITIES:
- Supervise the registered nurse team and case manager team.
- Support quality initiatives at Tepeyac through engaging and supporting staff working on quality improvement activities, facilitating QI (Quality Improvement) projects/teams as assigned, and assisting with the implementation of the annual Quality Plan and Risk Management Plan in coordination with the Quality, Risk, and Compliance Program Director.
- Monitor population health and clinical quality measures to identify and report on needs/desired outcomes for designated patient populations.
- Participate in departmental/cross-departmental quality improvement initiatives which support improved service delivery efficiencies, achievement of desired clinical outcomes, and/or enhanced patient experiences.
- Utilize patient registries to support the coordination and management of patients by care teams.
- Provide outpatient clinical nursing care within the RN scope of practice, either as part of a co-visit model with the primary care provider or independently.
- Provide comprehensive nursing assessments for referred patients, identifying barriers to achieving optimal outcomes and appropriate utilization of health system resources.
- After comprehensive nursing assessments, the RN/CM Lead will join with patients in their care plan development, creating appropriate and realistic, patient-centered interventions with cultural humility as a cornerstone. Responsible for service delivery with focus on entry and retention into medical care, treatment adherence, harm reduction, and chronic disease self-management.
- Coordinate client care with medical, mental/behavioral health, external substance abuse programs and other external providers, and ensure patient needs have been met to the best of our ability.
- Monitor client stability and self-sufficiency to provide other services as identified.
- Provide referrals and link clients to agencies, community, and government services.
- Communicate patient care plans with interdisciplinary team members and assist with coordination of integrated services.
- Ensure a safe environment for themselves, the patient and other staff members, to deliver safe and effective nursing care.
- Infection control audits/review of procedures.
- Support employee health programs.
- Medical emergency preparedness training and support.
- Act as a patient advocate that promotes the quality of health care delivered in the facility and always serve as a leader to promote best practices within the profession of nursing.
- Ensure medical record documentation is complete, thorough, timely, and accurate and in accordance with health center and nursing standards.
- Perform delegated tasks, activities, and functions that are consistent with prior educational preparation and within the scope of practice.
- Implement patient education based on established teaching plans and experience.
EDUCATION AND EXPERIENCE:
Minimum Required Education:
- RN licensure from an accredited nursing program required.
- Oral and written proficiency in English and oral fluency in Spanish required.
Minimum Required Experience:
- Five years of experience working as an RN required.
- One to two years of experience working as a case manager required.
- Prior supervisory experience required.
Preferred qualifications or experience:
- Experience in community/public healthcare preferred.
- Certification in Clinical Care Management preferred. (e.g., CCM (Chronic Care Management), ANCC (American Nurses Credentialing Center) RN-BC, ACM, etc.)
- Patient Navigation and/or Motivational Interviewing Training preferred.
REQUIRED SKILLS:
- Experience in community/public healthcare preferred.
- Certification in Clinical Care Management preferred. (e.g., CCM (Chronic Care Management), ANCC (American Nurses Credentialing Center) RN-BC, ACM, etc.)
- Patient Navigation and/or Motivational Interviewing Training preferred.

PAY RANGE AND COMPENSATION PACKAGE:
The salary range for this position is $72,000 to $100,000 annually, depending on years of experience and years of licensure.
