Job Objective:
- Provides direct patient care to primary care patients and support to a multidisciplinary health team working within the scope of the Advance Practice Registered Nurse (APRN) certified as a Family Nurse Practitioner. The APRN must demonstrates commitment to system values through customer focus and process improvements
Qualifications:
Education
- Required: Masters’ degree (MSN) in Nursing or Doctor of Nursing Practice (DNP)
Licensure/Certification
- Required: Active and unrestricted California licensure as a Registered Nurse and Nurse Practitioner; Board Certified as Family Nurse Practitioner (FNP) through ANCC or AANP; California Nurse Practitioner Furnishing number; DEA provider number schedule II -V; CURES registration; NPI Number; American Heart Association BLS
Experience
- Preferred: Three (3) years relevant experience providing patient care in a Primary Care setting
