Faith Hospice offers a unique opportunity for an experienced RN to join our dedicated interdisciplinary team. As a Hospice Response RN, you will play a crucial role in providing compassionate care to patients at the end of life, giving our RN Case Managers well-earned peace of mind that their patients and families are in good hands with the after hours and weekend team!

Responsibilities:
- Respond to crisis calls and patient/family concerns
- Provide admissions and end of life visits
- Provide after-hours support and care
- Work as an on-call RN and be paid for visits made/calls taken while on call OR work as a salaried shift RN
Choose your schedule:
- Full-time, weekdays only or include every other weekend
- Part-time, weekdays only, every other weekend, or every weekend
- Per diem, give us your availability to make visits and we can match it to our patients' needs
Location:
- Grand Rapids, covering a 35-mile radius around downtown Grand Rapids
Qualifications:
- RN licensure in Michigan
- Minimum 2 years of med/surg or acute care nursing experience
- Hospice or hospital experience preferred
- Passion for hospice nursing
- Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
