Job Description
Under collaborative agreement with a VNA Health Center physician, provides comprehensive medical evaluation, education and management for clients throughout the life cycle that present for routine preventive, acute and chronic disease health care services at the VNA Health Center clinic sites (on and off site clinics).
- Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment, provides education and referral (as indicated); and evaluates effectiveness of treatment, education and/or referral.
- Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral source when indicated.
- Orders, may perform and interprets the results of laboratory tests as part of the physical examination and health management. Reports communicable disease in a timely manner to the appropriate Health Dept. per applicable procedures and guidelines.
- Conception and contraception management per applicable protocols and state and federal family planning program guidelines for women presenting for family planning services. This partially includes as indicated: confidential family planning services for adolescents; pregnancy testing and options counseling; provision of emergency contraception; and consideration of all FDA approved contraceptive methods for women seeking contraception.
- Conducts health teaching and counseling during the physical exam, as appropriate, enabling the client to make informed choices about health and treatment options.
- Completes client clinical record documentation in a timely manner specific to the clinical services provided, according to applicable policies and procedures.
- Applies exposure control procedures as described in the VNA Health Center Infection control policy and Procedure Manual.
- Understands the medical emergency procedures and is familiar with the location of the emergency kit(s)/supplies in all clinic settings.
- Maintains professionalism and adheres to client confidentiality in all VNA Health Center practices.
- Participates in quality improvement activities, including, but not limited to: periodic clinic observation of clinical/lab skills by the medical director or designee; chart audits; outcome studies; client satisfaction surveys; clinic flow analysis; proficiency testing for CLIA; and quality management reporting and meetings.
- Familiarizes self and remains current with VNA Health Center policies, procedures and protocols. Assists in the review and development of standing orders, policies, procedures and protocols to be in compliance with state and federal guidelines, and up to date with current medical standards of practice.
- Provides consultative direction as needed for nursing, laboratory and clinic assistant staff.
- Provides staff and patient education.
- Identifies and participates in continuing education program/activities to enhance professionalism and to ensure quality of care.
- Participates in quality improvement/assurance activities.
- Provides on-call phone coverage as scheduled.
- Orders medications covered by scope of practice.
- Familiarizes self and remains current with VNA Health Center grant programs and regulatory guidelines. These include (but are not limited to) Family Planning; IL Breast and Cervical Cancer Screening Program; Bureau of Primary Health Care-Federally Qualified Health Centers; Advisory Committee on Immunization Practices-Indications, contraindications, administration and recommended schedule for vaccine preventable diseases; CDC-Treatment of Sexually Transmitted Diseases; IL School Code Health Requirements; IDPH Screening and Management of Lead Poisoning; and Dept of Child and Family Services-Mandated Reporting.
