Role Description
This is a full-time on-site role for a Case Management Nurse at Healogics, LLC. in Chicago, IL. The Case Management Nurse will be responsible for discharge planning, utilization management, nursing interventions, and medical case management to ensure optimal patient outcomes and efficient resource utilization.
Qualifications
- Discharge Planning and Utilization Management skills
- Nursing and Medicine knowledge
- Experience in Case Management
- Strong organizational and communication skills
- Ability to work collaboratively in a multidisciplinary team
- Active Registered Nurse (RN) license
- Case Management Certification (CCM) is a plus
- Minimum of 2 years of clinical nursing experience
