LICENSED PRACTICAL NURSE
Supervision and Direction:
The Licensed Practical Nurse receives general supervision and direction from the Executive Director and Delegating RN. The Licensed Practical Nurse is responsible for day-to-day support of the medication administration program, which is overseen by the Delegating Nurse and performed by the Certified Mediation Technicians. The LPN is responsible to ensure that resident services provide for the overall quality care of the residents. The LPN will comply with established policies and operational procedures. In the absence of the Executive Director or Delegating Nurse, the LPN will accept advice from the Community Manager or appointed designee.
Educational/Licensing Requirements:
LPN or higher
This position requires the candidate be a graduate of an accredited school of registered or licensed practical nursing and licensed in good standing. Must possess current:
- Unencumbered, active license to practice as an RN or LPN in Maryland or a compact state participating in the Nurse Licensure Compact. (The states participating in the Nurse Licensure Compact are Arizona, Arkansas, Delaware, Idaho, Iowa, Maine, Maryland, Mississippi, Nebraska, New Mexico, North Carolina, North Dakota, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin (Jan 1, 2005
- 54.1-3030, et. seq., Code of Virginia).
- CPR certification or acquire CPR certification within 60 days of hire.
Duties and Responsibilities:
The LPN’s responsibilities fall within the scope of Congregate Housing Services and include:
- Coordinating with the Community Manager resident service assignments and schedules to meet the needs of residents. Staff assignments will be made based on the resident’s selection of optional services, and the DRN and physician assessments.
- Recognizes situations in which resident requires additional help and coordinates efforts to acquire needed services with the Community Manager and Resident Services Coordinator, as well as the resident’s family members, outside community services and physicians.
- Support Delegating Nurse oversight of residents on the medication administration program by ensuring that:
- All physician orders are reviewed.
- New orders are sent to the pharmacy or treatment provider, and placed on the MAR.
- Appropriate paperwork accompanies residents to their physician appointments and review the physician paperwork upon return.
- Appropriate actions occur in response to changes in orders or new orders.
- There is accurate administration of ordered medications and treatments.
- MAR charting is accurate and appropriate, and coaches staff when corrections are needed
- All medications are filled in a timely and accurate manner, and reviews or checks CMT review of monthly medication cycle from pharmacy.
- Reports any problem or change in resident’s medication reaction to the physician and appropriate facility staff.
- Notifies Delegating Nurse, Community Manager, resident’s family or responsible party, and physician of any significant changes in the resident’s status.
- Document weekly resident notes and/or review CMT weekly notes regarding residents.
- In coordination with the Community Manager, monitor status of residents who are out at hospitals and rehabs, and ensure that a resident is determined to be safe to return to Edwards prior to their being discharged back.
- Obtain and review resident medical reports and discharge orders to determine if the facility can safely and properly manage the resident’s needs.
- Consult with family and physicians in discharge planning process.
- For residents on the medication administration program:
- Ensure that all medication orders are signed and sent to the pharmacy to have medications on-hand upon discharge.
- Remains constantly alert to help residents maintain or improve functional abilities within their present capabilities.
- Observes and records pertinent symptoms, profile and progress of the resident.
- Is knowledgeable of and complies with facility policies and procedures, forms and documentation.
- Is constantly aware of and alert to the problems of the elderly and disabled person, avoids any unnecessary noise, obstructing machinery and devices with which the resident could become injured, strictly observes the smoking restrictions and protects the resident from injury.
- Always maintains professional conduct that reflects credit to the profession and reflects the facility’s philosophy of quality care and service delivery.
- Participates and provides input on hiring, performance evaluations, terminations and progressive discipline of direct care staff.
- Monitors CMTs performance of health-related activities, including the identification of any significant gaps in the employee’s ability to function competently. Provides staff development and training as needed to ensure quality performance.
- Observes and protects the rights of all residents including the delivery of services in a dignified and caring manner.
- Keeps inventory at par level notifying Community Manager or designee of any needed supplies.
- Observes and protects resident rights including rights to confidentiality, privacy and dignity.
- Practices safety procedures and complies with established reporting of accidents and injuries.
- Performs any other related duties that may be assigned.
