Essential Job Duties:
1. Conduct home care assessments and reassessments using the Oasis tool.
2. Provide verbal reports of initial findings to supervisors.
3. Develop and implement individualized Plans of Care with the team.
4. Deliver skilled nursing care and specialized treatments as prescribed.
5. Initiate preventive and rehabilitative nursing procedures.
6. Evaluate effectiveness of nursing services provided.
7. Communicate changes in patient condition to supervisors promptly.
8. Submit daily records of visits and hours worked.
9. Participate in case management conferences and continuing education.
10. Educate and counsel patients and caregivers on nursing care needs.
11. Document services rendered and patient responses to treatment.
12. Ensure compliance with policies and procedures.
13. Attend in-service training and contribute to Performance Improvement.
14. Orient and supervise home health aide staff.
15. Ensure confidentiality, infection control, and compliance with Advance Directives.
16. Coordinate total patient care in collaboration with supervisors.
17. Respond to emergencies for all residents in a timely, professional manner.
18. Perform additional duties as assigned.
