Overview
Position Overview: The Enrollment Outreach Registered Nurse (RN) is an integral member of the Intake, Enrollment, and Marketing Team. This RN is responsible for conducting clinical assessments, completing necessary documentation for the intake process, and providing clinical support to the Marketing Department. They ensure that initial and annual clinical assessments are submitted to the State Administering Agency (SAA) to determine eligibility for the PACE program.
Key Responsibilities:
- Conduct home visits to perform clinical assessments as part of the pre-enrollment process, either independently or alongside the Intake Coordinator.
- Collaborate with the Intake Coordinator to complete the Intake Prospect Sheet for each home visit.
- Provide clinical information to the Interdisciplinary Team (IDT) and contribute to the completion of the initial Minimum Data Set (MDS) and provisional care plan.
- Serve as the liaison between PACE and the pre-enrollee’s existing home services.
- Submit the initial MDS assessment and annual re-assessments to the SAA to confirm and maintain PACE clinical eligibility.
- Oversee the reassessment process for PACE eligibility and recertification.
- Ensure timely completion of MDS assessments and data entry into the Electronic Medical Record (EMR) system.
- Maintain an organized tracking system for the MDS process.
- Communicate effectively with the State Administering Agency.
- Maintain regular communication and updates with clinical partners.
- Provide clinical outreach and support to community referral sources.
Requirements:
- Home care and/or intake experience with SCO or PACE program
- Strong written and verbal communication skills in English
- Experience with completing assessments (MDS)
- Computer knowledge required
- Registered Nurse, Mass license
- CPR certified
- First Aid certified
- EPIC experience preferred
- Bilingual preferred
