We have an exciting opportunity for a Nurse to join our team of talented professionals.
As a Nurse in the Assertive Community Treatment (ACT) Program, you will be responsible for the following:
- Deliver nursing and psychosocial services utilizing person-centered principles consistent with the client’s Individualized Plan of Service.
- Conduct home visits and community outreach to help engage clients, their family members, guardians and other supporters in the recovery process.
- Work collaboratively with members of the ACT team to deliver case management and advocacy services to all clients.
- Maintain regular contact with clients, ACT team members and other providers to coordinate service delivery efforts.
- Coordinate with the client’s primary care and other health care providers to ensure continuity of care.
- Medication monitoring, assistance, administration and education.
- Medication disbursement.
- Crisis intervention.
- On-call responsibilities. Will be available 24/7 to respond to client calls as needed. Regular working hours are 8:30 am to 7:00 pm Monday thru Thursday and 8:30 am to 5:00 pm Friday; however, due to the needs of the consumer and the program additional hours may include evenings and weekends.
- Adhere to current ACT fidelity standards.
- Document all face-to-face service activity and phone contacts within expected timeframes.
- Respond in writing to routine chart reviews and audits.
- Participate in regularly scheduled clinical supervision, case consultations and team meetings.
- Perform within acceptable standards of productivity.
- Maintain appropriate state licensure or certification;
- Complete 24 hours of program and/or position specific training on an annual basis.
- Follow a professional code of ethics.
- Maintain appropriate professional ethics and boundaries;
- Abide by all Guidance Center policies and procedures;
- Other duties as necessary or requested.
- Conduct direct service as necessary and required.
- Ensure documentation is completed in a thorough and timely manner consistent with the policies and procedures of The Guidance Center, the CAC and all requirements of the CAC and funding sources
- Complete 25 hours of staff development training annually.
- Other duties as assigned.
Requirements
To qualify for this position, you will need the following:
- Must have an Associate’s degree in Nursing. Bachelor’s degree in Nursing preferred.
- Must possess and retain a current license as a Registered Nurse in the State of Michigan.
- Must have CPR and First Aide Certification.
- Must have two years of experience (may include time spent in fieldwork or internship) treating or working with people who have been diagnosed with mental illness.
- Must meet the requirements for Qualified Mental Health Professional within 120 days of hire.
- Must possess a valid driver’s license and a good driving record. Must obtain a Michigan Chauffeur License within 30 days of employment.
- Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain clinical relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative.
Starting salary $71,911.13 and increases with relevant experience.
