This is a per diem role for a Case Management Nurse at Adult Healthcare Services in Appomattox and surrounding counties. The Case Management Nurse will be responsible for coordinating patient care, collaborating with healthcare providers, and ensuring efficient transitions through the healthcare system. This is a great job opportunity to earn extra money. We are looking for area in Charlotte County, Campbell County, Appomattox County and Amherst County.
Qualifications
- Active Registered Nurse (RN) license
- Experience in case management or care coordination
- Strong communication and interpersonal skills
- Ability to collaborate with a multidisciplinary team
- Knowledge of healthcare regulations and patient advocacy
